Once you have registered as a new patient and have a confirmed appointment, you will receive an email and text link prompting you to complete your intake forms via a secure, HIPAA-compliant telehealth platform called Tebra. I ask that all forms be completed at least one week in advance of your first scheduled appointment. Please feel to reach out if you have any questions regarding completion of the forms.
